I figured out it's not creating a folder for users, had to do this manually.
How do I enable it to create user folders automatically on signup?

The first thing to make sure is your server is configured to be able to accept uploads of the file size you are trying to upload.
Secondly, make sure you aren`t running ModSecurity, which can cause this error. Failing that, check your server error logs.
